Job Description

Responsible for the direction of the clinical hospice program to assure patient and family services are provided at high quality levels consistent with professional standards and goals. Assures compliance with applicable local, state, federal laws and regulations and accrediting bodies.

Duties and Responsibilities

• Supervises staff and provides staff performance evaluations/counseling/discipline.
• Schedules, maintains and facilitates interdisciplinary team meetings to review and revise patient/family care plans, exchange of information, problem solving and assuring services meet patient/family needs and goals.
• Develops and promotes programs for continuing staff support, inservices and education.
• Assures complete patient medical records for compliance with regulatory agencies.
• Interviews, hires and provides orientation and training of staff.
• Directs quality assessment and improvement opportunities.
• Plans, coordinates and delivers care to patients and families as appropriate.
• Participates in on-call coverage.
• Monitors direct patient care costs and implements staff education to deliver cost effective, high quality and appropriate care to patient and families.
• Directs and is responsible for Safety Committee, QAPI Committee and projects, and serves on the Professional Advisory Committee.
• Works with improvement team to set measurements, outcomes, and goals and assists team until improvement and desired outcomes are achieved.
• Organizing & directing the organization’s ongoing operations to assure availability and provision of care and services.
• Implements Governing Body directives and organizational policies and procedures.
• Recruits, employs and retains qualified personnel to maintain adequate and appropriate staffing levels, ensuring interdisciplinary care is provided.
• Insures adequate staff education and development including orientation, in-service, continuing education and competency testing.
• Complete performance evaluations in accordance with organizational policies.
• Manages operations in accordance with established fiscal parameters to insure financial stability.
• Plans, develops, implements, administers and evaluates programs.
• Represents the organization’s mission and vision to other groups, organizations and the general public in a professional manner.
• Insures the accuracy of public information materials.
• Informs the Governing Body staff of current organizational, community and industry trends.
• Evaluates Hospice services and personnel using measurable outcomes and objectives.
• Conflict and complaint management and resolution.
• Directs staff in performance of their duties including admission, discharge, transfer, revocation and provision of service to patients.
• Ensuring that hospice personnel stay current with clinical information and practices, and local and national issues and trends.
• Insures appropriate staff supervision during all service hours.
• Monitors service utilization to insure delivery of comprehensive care.
• Insures services provided by other agencies are authorized by hospice.
• Insures that standards of ethical business and clinical practice are maintained.
• Insures that all care is provided with respect for patient and family rights.
• Reports all grievances/complaints made by patients or families to supervisor per policy.
• Reports all allegations of patient abuse, and/or misappropriation of patient property.
• Assists with other duties as assigned.

Required Education, Experience and Licensure

• Currently licensed as a Registered Nurse in good standing with the State Board of Nursing.
• Bachelor’s degree in Nursing and Hospice and Palliative Care Certification preferred.
• Obtains CHPN certification within one year of employment or at the discretion of Executive Director.
• Minimum 1 year management experience; 5 years nursing experience; hospice experience highly preferred.
• Adheres to the Ethical and Religious Directives for Catholic Health Care Services outlined by the United States Conference of Catholic Bishops, while performing job duties.
• Refrains from participations in practices in direct conflict with the Ethical and Religious Directives for the Catholic Health Care Services outlined by the United States of Catholic Bishops, while performing job duties.
• Respects and uphold the religious mission of the Saint Jude Hospice, while adhering to the directives. Maintains professional standards and promotes the institution commitment to the human dignity and common good. (ERD#9)
• Participates in education in the regards to the Ethical Religious Directives and the mission of Saint Jude Hospice.

• Must operate a car in a safe manner and maintain a valid driver’s license.

Travel and Working Conditions

• Travel necessary on a daily basis.
• Exposure to disagreeable conditions including odors, infection, illness, or physical contact from disruptive patients in patient homes/living facilities.
• Principal contacts include: patients, families, pets, physicians, office staff, field staff and general public.
• Physical demands include: lifting 25-50 pounds; walking, standing, stooping, reaching, talking, hearing, seeing, smelling, depth perception, color vision.
